Primary Care Network

A primary care network is a group of practices who work together to focus on local patient care. They've been around since 2019 and 99 percent of practices across England now work as part of one. South Queen Street Medical Centre is part of the Morley Primary Care Network. 
 
With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they work together on certain services or issues.

Community nurses

The district nursing team works across the whole of the practice geographical area. They have close links with social services and work collaboratively with other healthcare professionals. They visit people in their own homes and residential homes, assessing patients’ individual nursing needs and providing appropriate treatment and care.

The role of the district nursing team covers palliative care, support to carers, ulcer management and follow-up of medical and surgical patients discharged from hospital. Their role encompasses health promotion in all aspects working closely with our GPs, nurse practitioner and practice nurses.

Health visitors

Our health visitors are qualified nurses with specialist training in family and child health, health promotion and education with practical support and advice provided in a host of diverse situations. Health visitors work with individual families in their own homes as well as working to promote the health of the community as a whole.
